6. Morning or evening hunt. (Evening/shot was made at 3:04 PM)
7. Shot distance. (25 yards)
8. Distance animal traveled. (200 yards)
9. Number of points (if applicable) or if a turkey give the spur and beard length, weight etc. (Main frame 10pt with a split brow tine for a total of 11 scoreable points)
10. Interior Spread of Antlers (if applicable). (18 3/4" inside spread)
11. Animal age and weight (live or dressed weight) if known. (258lbs.- live weight)
12. Shot placement. (heart, double lung, etc). (Double lung)
